Supported Independent Living (SIL) helps you live life on your terms. Through SIL, you have access to 24/7 personalised support designed to assist with everyday tasks, such as cooking, cleaning, and personal care, while also helping you build confidence and learn new skills for greater independence.

 

Whether you want to manage your own home, participate in community activities, or develop life skills that matter to you, SIL provides the flexibility and choice you need.

What support will I receive?

Our Supported Independent Living service is tailored and flexible to what you need, dependent on your NDIS plan or alternate funding.

We are not an SDA or housing provider. The house, unit or apartment where you choose to live is your choice. It might be your own home, a Housing SA property, a private rental or a home managed by a community housing association. You, as the tenant, will sign the rental agreement with your housing provider. Cara, as your SIL provider of choice, will provide staff to work alongside you in your home.

We know that who you live with is very important. We spend a lot of time matching people who are like-minded and share similar interests and goals.

Cara’s Client Services Managers (CSM’s) are our personalised point of contact for Cara clients, their families, support coordinators, and further circle of supports. Think of our CSM’s as your partner for your journey with Cara and the NDIS.

Your Step-by-Step Journey to a Supported Home

1. Initial Contact

Contact the Cara Intake team via telephone on 08 8347 4588, via email or using the online form.

2. Registration

Complete the registration of interest form.

3. Needs Evaluation

We will evaluate your needs and identify if Cara is the best provider for you.

4. SDA Eligibility Assistance

If you’re not already eligible for SDA, we can help you to connect to Occupational Therapists to get Functional Capacity Assessments to assist with the SDA eligibility process.

5. Intake Form Completion

If we’re the right provider, you will be asked complete an intake form (this is different to the registration of interest form you complete earlier in this process). We can then meet you at your home, a local café or welcome you to our head office at Hindmarsh to take you through the next steps of the process.

6. Funding and Matching

We will assess your funding, support ratios and start matching you with the right housemate and service.

7. Roster Approval

Once we’ve found the right match, we will provide you and/or your guardian/support coordinator our Roster of Care Tool and Template for approval. We will then ask you to submit it to the NDIS.

8. NDIS Assessment

The Roster of Care Template and Tool will be considered by the NDIS, together with all other information and evidence they have about your support needs. The NDIS will assess your home and living support needs and fund these accordingly. We can start planning and completing your move once you have sufficient funding in your plan for the support you require. Our team will support you with your transition and will be there every step of the way. We want to ensure that this process is well planned and considered, to ensure you to feel comfortable with this important transition.

9. Transition Support

We will work closely with you and your support network to complete the move to your new home. This will include identifying a move date, finalising tenancy and board arrangements, rostering your support staff and setting up your home environment.
