Transition means moving from where you are now to your next stage of life. When you turn 18, you’re considered an adult and you’re able to do lots of new things.

You might be transitioning in to a job, into study, starting Day Options, or you might want to move in to your own home with Supported Independent Living (SIL). It can be a really exciting time.

Cara can work with you to make this transition easier for you – we offer individualised support in your transition to the next stage in your life.

How does it work?

Staff from Cara’s Specialist Services team will work with you, your
family, support staff and case workers or support coordinators.

Working as a team, we’ll make sure that your goals in your National Disability Insurance Scheme (NDIS) plan are met.

Cara can work with you to strengthen the areas of your life that are important to you.

We can help you identify the NDIS supports you might need using core, capital and capacity-building funding.
